Founder, Diane Pagan ANP-BC

Diane started Senior Moments Care from the ground up in 2023, after years of providing housecalls to older adults who deserved more time and attention than a traditional office visit allows. What began as one provider seeing patients at home has grown into a full team and a dedicated center, built around the same belief Diane started with: that older adults and their families deserve care that's thorough, unhurried, and genuinely personal.

She is Hospice & Palliative Care Certified, Care Management Certified, and board certified as an Adult Nurse Practitioner. In 2011, she received a Healthcare Hero award for her work providing housecalls at Montefiore. Her work has since been featured in The Examiner, Westchester Magazine, and recognized by Northwell Health.

Virginia Dronzek

Chief Operating Officer

Virginia is dedicated to creating a welcoming, compassionate environment for patients, caregivers, and staff. She works closely with the Senior Moments Care team to streamline operations, enhance communication, and ensure that every aspect of the practice reflects our commitment to excellence.

Eileen Kennedy

Director of Community Engagement & Programs

Eileen develops and coordinates the center’s quarterly social wellness calendar, bringing together educational opportunities, social activities, and meaningful programs designed to support engagement and overall well-being.

Danielle Sinanaj, FNP-BC

Family Nurse Practitioner

She brings a diverse clinical background spanning geriatric primary care, critical care, and wellness medicine. She has provided primary care to older adults in the nursing home setting for the past two years, following four years as a Cardiac Intensive Care Unit nurse and additional experience in aesthetic medicine.

Nurse Practitioner

Eleni believes in a holistic approach to healthcare, thoughtfully incorporating evidence-based complementary therapies when appropriate. As an end-of-life doula, she is especially passionate about hospice and palliative care, helping patients and families navigate every stage of the aging journey with compassion and dignity.

With more than 20 years of experience, Yenny is passionate about providing compassionate, personalized care to older adults. She believes every patient deserves to be treated with dignity, respect, and kindness, and is committed to building trusting relationships with both patients and their families.

Carla coordinates appointment scheduling, arranges transportation, manages incoming documents and faxes, and helps keep the practice running smoothly. She strives to make each patient interaction helpful, organized, and as stress-free as possible.

Facilities + Operations Support

Scott provides hands-on support throughout the Geriatric Center of Excellence, helping with facility improvements, maintenance projects, painting, furniture assembly and moving, and the countless behind-the-scenes tasks that help keep the center welcoming, safe, and running smoothly.

Bill Julian

Transportation Coordinator

Bill provides safe, reliable transportation for Senior Moments Care patients traveling to and from medical appointments and the Geriatric Center of Excellence. Serving Somers, Katonah, and neighboring communities, Bill helps ensure patients can access the care and services they need with comfort and peace of mind.

Meet Sammy

Sammy is Senior Moments Care’s lovable robotic pup and office companion, bringing joy, affection, and a little extra warmth to our patients and visitors.

For those who would enjoy a little more time with Sammy, he is available for weekend companionship visits at home—a special opportunity to enjoy his playful personality and comforting presence beyond the SMC office.
