Diane Pagan, an adult nurse practitioner and director of Westmed’s Complex Care Program, oversees some of the healthcare system’s most challenging patients. “They have multiple chronic conditions, and their conditions could be complicated by socioeconomic factors, or they could be homebound,” she explains.
