Yes. Care coordination is an important part of what we do. We help bring together information from specialists, hospitals, pharmacies, home-care providers, and other members of your healthcare team.

Yes! Many of our wellness, educational, caregiver, social, and cognitive programs are available even if SMC is not your primary medical provider.

SMC offers cognitive assessment, cognitive wellness programs, caregiver education and support, and activities designed specifically for individuals experiencing changes in memory or thinking.

We believe caregivers need care, too. SMC provides education, resources, support, training, and opportunities for caregivers to connect, recharge, and feel more confident in their caregiving role.

Transportation assistance may be available for eligible SMC patients participating in appointments or programs at the Center. Please speak with our team about availability and scheduling.
